Background
The U.S. Food and Drug Administration (FDA) traditionally has kept confidential significant amounts of information relevant to the approval or non-approval of specific drugs, devices, and biologics and about the regulatory status of such medical products in FDA’s pipeline.
ObjectiveTo develop practical recommendations for FDA to improve its transparency to the public that FDA could implement by rulemaking or other regulatory processes without further congressional authorization. These recommendations would build on the work of FDA’s Transparency Task Force in 2010.
MethodsIn 2016-2017, we convened a team of academic faculty from Harvard Medical School, Brigham and Women’s Hospital, Yale Medical School, Yale Law School, and Johns Hopkins Bloomberg School of Public Health to develop recommendations through an iterative process of reviewing FDA’s practices, considering the legal and policy constraints on FDA in expanding transparency, and obtaining insights from independent observers of FDA.
ResultsThe team developed 18 specific recommendations for improving FDA’s transparency to the public. FDA could adopt all these recommendations without further congressional action.

The abundance and distribution of echinoderms in nearshore hard-bottom habitats near Anvers Island, western Antarctic Peninsula

Echinoderms are well represented in nearshore hard-bottom (< 100 m depth) habitats along the Antarctic Peninsula where they are presumably important contributors to benthic production, carbon flow, and determinants of community structure. The present study assesses the densities of echinoderms at shallow depths (2–15 m) at five sampling sites within three kilometres of Anvers Island on the central western Antarctic Peninsula. The asteroids Odontaster validus, Granaster nutrix, Lysasterias perrieri and Adelasterias papillosa, two ophiuroids in the Amphiuridae, the holothuroids Psolicrux coatsi and Psolus carolineae and one representative of the Cucumaridae, and the regular echinoid Sterechinus neumayeri were enumerated. Mean total echinoderm densities were high (34.9 individuals m-2) and ranged from 21.9 individuals m-2 for asteroids to 2.7 individuals m-2 for holothuroids. With the exception of a positive relationship between the abundance of the regular echinoid Sterechinus neumayeri and the biomass of the brown alga Himanthothallus grandifolius, no significant relationships were found between the abundance of asteroids, ophiuroids, or holothuroids and two species of brown algae or three algal ecotypes. The present study indicates nearshore hard-bottom echinoderms are important in the carbon cycle and their inherent vulnerability to ocean acidification may have community-level impacts.

The purpose of this study was to investigate the phenomenon of spousal caregiving following the institutionalization of elderly husbands. Theoretically, the study was informed by the interpretive perspective in sociology and the concept of career. Methodologically, the study employed a longitudinal and prospective design and combined quantitative and qualitative approaches. The data were drawn from a larger study designed to explore the transition to quasi-widowhood. This article reports on one aspect of spousal caregiving following the admission of husbands to a long term care setting, i.e. the performance of tasks. In this study, wives carried out a substantial number and wide variety of tasks related to personal, instrumental, relational and recreational care. They viewed visiting as their most important task but also highly valued providing companionship, love and support, doing little things for their husbands and monitoring their health and well-being. Typically, wives carried out tasks they viewed as falling within their domain of responsibility. Task performance occurred within a context of meaning that served to explain, in part, their continued involvement in caregiving following the admission of husbands to a long term care setting. Implications of this study point to the need for collaboration in caregiving between wives and staff, to support and enhance the quality of care provided by elderly wives who are coping with the institutionalization of their husbands.

As Canada's elderly population grows, an increasing number of seniors will require care as they face the end of life. Relatively little research attention, however, has been given to end-of-life issues, including those related to treatment and care for persons who are both old and dying. Families and health care providers have little direction and guidance to ensure an optimum quality of life for seniors prior to death and during the period of bereavement. This article presents an abbreviated summary of the literature regarding end-of-life issues and challenges for seniors who face a life threatening illness and are at, or near, the end of their lives. The article is also informed by a series of focus groups with elderly and other informal caregivers regarding the giving of help and getting support while caring for persons who are dying. It concludes with a response to this evidence by making a series of recommendations aimed at ensuring optimal end-of-life care for seniors who are dying and their families.
Outbreak of Multidrug-Resistant Serratia marcescens Infection in a Neonatal Intensive Care Unit

Background.
Serratia marcescens causes healthcare-associated infections and significant morbidity and mortality in neonatal intensive care units (NICUs). We report the investigation and control of an outbreak of multidrug-resistant (MDR) S. marcescens infection at an NICU.
Methods.An outbreak investigation and a case-control study were undertaken at a 36-bed NICU in a tertiary care hospital in Baltimore, Maryland, for the period from October 2004 through February 2005. The outbreak investigation included case identification, review of medical records, environmental cultures, patient surveillance cultures, personnel hand cultures, and pulsed-field gel electrophoresis (PFGE). The case-control study included case identification and review of medical records. Infection control measures were implemented. Eighteen NICU neonates had cultures that grew MDR S. marcescens during the study period. The case-control study included 16 patients with the outbreak strain or an unidentified strain of MDR S. marcescens and 32 control patients not infected and/or colonized with MDR S. marcescens, treated in the NICU for at least 48 hours during the study period.
Results.PFGE analysis identified a single strain of MDR S. marcescens that infected or colonized 15 patients. Two patients had unique strains, and 1 patient's isolate could not be subtyped. An unrelated MDR S. marcescens isolate was recovered from a sink drain. Exposure to inhalational therapy was an independent risk factor for MDR S. marcescens acquisition after adjusting for birth weight. Extensive investigation failed to reveal a point source for the outbreak.
Conclusion.A single epidemic strain of MDR S. marcescens spread rapidly and threatened to become endemic in this NICU. Transient carriage on the hands of healthcare personnel or on respiratory care equipment was the likely mode of transmission. Cohorting patients and staff, at the cost of bed closures and additional personnel, interrupted transmission and halted the outbreak.

Summary
In recent years there has been an increasing recognition of the association between marital conflict and children's adjustment (Fincham, 1998; Grych & Fincham, 1990). In spite of the increase of attention to the effects of familial conflict on children's functioning, there has been little attention devoted to children's competence with peers. The goal of this chapter is to address this relatively unexplored issue of the links between marital conflict and children's peer relationships.
Importance of Examining Links between Marital Conflict and Peer Relationships
Understanding children's social competence with peers as an outcome of marital conflict is critical for several reasons. First, the correlates and predictive associations of social incompetence (including truancy, school dropout, and criminality) have been well documented (Parker & Asher, 1987). Second, as Wasserstein and La Greca (1996) found in their investigation of marital discord and peer support among children in middle childhood, children's close friendships moderate the negative effects of marital discord. This finding highlights the importance of understanding the links between marital discord and children's peer relationships. Third, although most past research in this area has used unitary measures of child adjustment (e.g., internalizing or externalizing problems), these global evaluations need to be complemented by more specific indices in order to identify particular outcomes that may be more proximal or distal to particular aspects of marital conflict (Grych & Fincham, 1990). These global problems may, in fact, be associated with peer relationships, but there is little research that has investigated how children from families high in marital conflict are perceived socially by their peers and teachers and their level of acceptance in peer contexts.

To rely on a system of drug administration based on a transcribed drug list is to court disaster. The high incidence of errors of transcription has been documented by Crooks (1964) (1), Wallace (1965) (2), Watt et al. (in press) (3).
